Introduction to pH testing and GERD

Gastroesophageal reflux disease, or GERD, is a digestive disorder in which frequent bouts of acid reflux are experienced by the patients and it causes chronic irritation of the esophageal lining. The lower esophageal sphincter, which is a stomach valve doesn’t close properly or open too often, causing the acid produced in the stomach to move back into the esophagus. An esophageal pH test is necessary to diagnose the above condition.

When to consult a doctor?

Experiencing symptoms mentioned below two or more times in a week:

  • Heartburn
  • Regurgitation
  • Bloody or black stools, bloody vomiting
  • Nausea
  • Weight loss for no reason
  • Hoarseness or chronic sore throat

How is GERD diagnosed?

For confirmation of GERD, several tests can be performed, pH testing or monitoring is one of the diagnostic and confirmatory methods for GERD as the symptoms could be quite misleading and several medical conditions lead to the above-mentioned discomfort.

Is the pH test scary?

Absolutely not! It measures the frequency of stomach acid entering the esophagus and the duration for which the acid stays. Two types of the test form exist:

1. Classic Catheter Method

Transnasally, the catheter is inserted and fixed using tape. The pH sensor is positioned in the distal esophagus, usually, 5 cm above the manometric position (with reference to the proximal border of the lower esophageal sphincter) in adults and in children and infants the position could be determined under fluoroscopic guidance (with reference to the diaphragm, since manometry is nearly impossible to perform in this group).

The pH monitoring begins on the monitor that is strapped onto your waist or shoulder.

For 24 hours the acid levels are measured, though the first 16 hours are considered more accurate.

2. Recent Catheter free or Wireless Capsule Method

In this radiotelemetry capsule (containing antimony pH electrode with internal reference) is placed 6 cm above the endoscopically determined gastroesophageal mucosal separation in the esophageal mucosa using a delivery system after which the latter is removed.

pH data recording from the distal esophagus is done every 5 seconds.

Acid levels are measured for 48 to 72 hours.

Your role in the test!

Note your symptoms along with the food you uptake for the next 24 hours in the catheter method and for the next 48 to 96 hours in the wireless method.

Go on with your usual routine and eat what you eat normally.

How can you prepare for the test?

Generally, certain medications need to be discontinued such as antacids six hours before the monitoring period, proton pump inhibitors seven days before the monitoring period, and H2 blockers 48 hours before the monitoring period but consult your doctor regarding the drugs you are on and follow the advice as per se.

Are risks associated with the test?

It is an extremely safe procedure and no risk factors are associated with it.

Feeling of gagging, when the tube is inserted can be experienced.

A sore throat in the wireless method from the endoscope could be experienced and extremely rare, a tear in the lining of the esophagus.
